Becoming a Patient

There are several important steps to becoming a patient in The Nebraska Medical Center transplant program. The first is to be referred to our transplant center. Patients may be referred in several ways:

Self Referral/Friend or Family
Patients may refer themselves to one of our transplant programs, or they can be referred at the suggestion of a friend or family member. To begin the process, please contact one of our transplant coordinators who will answer questions and complete a referral form. Contact Us

Medical Professionals
A medical professional, such as a physician, may refer a patient to our transplant center. Physicians and Medical Professionals please click here for information on referring a patient.

Insurance Carriers and Case Managers

Social Workers
A social worker may refer a patient to our transplant center. Social Workers please click here to resource further information.

Dialysis Unit
Dialysis patients may be referred by their dialysis unit to our Kidney/Pancreas transplant program. Dialysis professionals click here for information on referring a patient.

The Next Steps:

  • After the referral, a Pre-Transplant Coordinator will contact the patient to gather information and answer any questions.
  • An evaluation packet will be sent to the patient’s home. The packet contains information about the transplant process and procedure, living donor information, information about our Transplant Center and questionnaires to be completed and returned.
  • Medical releases are included in the packet so the Transplant Center can obtain necessary records from physicians and/or hospital.
  • Once we have the questionnaires and medical records, the Transplant Team will review the information and contact the patient to schedule a time to come to The Nebraska Medical Center and complete the transplant evaluation.
